Note : In the absence of a non-contamination form, items will not be accepted pending receipt of this document. In the case of soiled, uncleaned equipment, a cleaning service may be offered.

This form must be packed with the goods when they are returned to MATISEC. Goods must be marked on the outside with the RA number.
